Thematic Area 1: “Dementia and Frailty Service Delivery Models for affected patients and their carers” by EHTEL
Focused on interventions to support people already affected by frailty and dementia and their carers. Addresses the complexity of dementia and frailty, highlighting the limitations of traditional clinical pathways and emphasizing efforts to extend autonomy and improve quality of life.

Thematic Area 2: “Skills to facilitate the Service Delivery Models.” by DEPAN
Highlights essential enabling factors for effective service delivery in dementia care, especially the role of a skilled workforce and the importance of digital health technologies.

Thematic Area 3: “AI-based Prevention of Cognitive Decline.” by CERTH
Focuses on strategies and technologies (like AI, Virtual Assistive, and AAL) to prevent cognitive decline in individuals at risk of dementia.
